Question: Suddenly roars like truck

Purred like kitten until 2-3 months ago. Now when in drive roars like a truck to the point of hurting ears at times. Worse when cranked first time of day and calms down some as car warms up and is in motion. Very obvious when idling and not heard in park or reverse. Been told there is no exhaust problem
